Submitting a request does not guarantee a clinic will be run at your facility - it merely provides the office and our supervisory officials information to help us understand your needs and how we may accommodate them.

The application must be submitted to our office via the form linked on this website for us to begin the planning of your clinic.  This information must be submitted no less than three weeks before the earliest clinic date requested.  The dates to request must be after 12/1/23.

Clubs must read the guidelines on the application and adhere to them. 

• Three weeks prior to earliest desired clinic date - application must be submitted to the CEVA office via the form on this site.
• Have the player organization template completed for the officials who are running your clinic on the date you requested.  Template is available on the application.
• Make sure all players have numbers who are on the court playing during the training.
